Before you can pull form code, you need to create the necessary Source Values, Segmentation Fields, and External Events within the Listrak Application.



  1. Navigate from the home menu to Contacts.
  2. Go to Segmentation.
  3. Then Modify Existing Field Group.
  4. Click on the Source Field Group. If one does not exist, click the Create Field Group button and create a new field group titled “Source”. 
  5. Once you’ve saved it, it should open the Source Field Group. If not, click on it.
  6. Type the name of your new source field under the Field Name column. For this example, we’re going to be mapping a form on an email signup page. Let’s use “EmailSignup” as the name of the Source field.
  7. Under Data Type, select CheckBox from the drop-down. Click the green floppy disc icon to the left of the field to save when you’re finished.


To collect additional user information beyond email address, you need to set up the corresponding segment fields within the Listrak application. For our email signup form example, we’re collecting multiple fields as shown in the sample form below:

  1. Gender
  2. First Name
  3. Last Name
  4. Zip Code
  5. Birthdate (month, day, and year)

Create these fields by following the below steps:

  1. Navigate from the home menu to Contacts.
  2. Go to Segmentation.
  3. Then Modify Existing Field Group.
  4. Click on the CustomerInfo link under Field Group. If no such field group exists, click the Create Field Group button and create a new field group titled “CustomerInfo”.
  5. Type the Field Name of your new profile field. For this example, it would be “FirstName”. Omitting spaces is recommended.
  6. Under Data Type, select Text from the drop down. Click the green floppy disk icon to the left of the field name to save.
  7. Finish creating all fields with the correct field type.

NOTE: when creating ZipCode, we recommend creating it as a text field instead of numeric to accommodate any dashes and international users with alphanumeric postal codes. Also take note of the Size column, which, when using a comment or text field, will dictate the character (and line) limit for the field.


Create external events to trigger your automated campaigns.

  1. Navigate from the home menu to Authoring.
  2. Go to Listrak Conductor.
  3. Then External Events.
  4. Click New Event.
  5. For Event Name, use the same name that was used to create the Source field, using no spaces.
  6. Click Create Event to save your event. Apply a group, if applicable. 


Now that all values are created, you may generate HTML form code.

1. Navigate from the home menu to Support.
2. Go to Code Support.
3. Then Subscribe Contact Form.
4. Under the “Select Segment Field Groups To Include” heading, check the box for Source.
5. If you are collecting additional customer information, make sure to check CustomerInfo.

6. If you wish to define a confirmation/ "Thank you for subscribing" page URL, enter this in the 'After submission, redirect to' field. If you do not have a confirmation page available, the standard success screen from Listrak displays after the form submits.

7. If you would like to send subscribers a double  opt-in email, select Enable Double Opt-In. When this is enabled, an automated email will ask the subscriber to confirm their subscription before being added to your list. This is optional.

8. DO NOT CHECK YES TO SEND WELCOME EMAIL. It will NOT deploy your branded Listrak welcome series (if purchased). It will instead send an unbranded, plain text system message.

9. Next, choose the external event you just created in the Execute External Events Option.
Note: The event will not run if Enable Double Opt-In is set to Yes.

10. When you're finished, hit the green Update button at the bottom of the screen.


Although the HTML will be functional as-is, we highly recommend to first edit the code to only contain the information you need. The screenshots below show the code in Adobe Dreamweaver for editing, but you can also use notepad or a similar external code editor. 

  1. Copy the raw code from the Subscribe Form window in Listrak and paste it into your external code editor. You will immediately see that there are a few things in the code that aren’t necessary for this form. For the rest of this process, we will be deleting what we don’t need.



2. Start at the top of the <table>. Delete the ‘Please fill out this information’ row. Be sure to start with the beginning <tr> tag, and select everything until the ending <tr> tag.


3. Repeat this process until you are left with the rows for email address, the source you created (e.g. AccountCreate), any necessary customer fields (first name, birthdate, zip code, etc.), as well as the sign up button

4. DO NOT DELETE THE FORM OR INPUT TAGS AT THE TOP OF THE PAGE. Those elements are what will allow your form to work.

5. After you’ve finished deleting what you don’t need, your form should look something like below:

6. Next, find the <tr> row that defines your source checkbox, e.g. AccountCreate. Select the first <label> tag, and highlight everything until you hit the </label> tag (illustrated below). Delete this information.

7. Finally, find the value="off" attribute at the end of this same section of code. Change "off" to "on". This will allow the field to be hidden on the form, but will still pass the correct source value when someone subscribes.

Now you're finished - the code is ready! You can now save your text file locally on your machine as filename.HTML, and double-click the file to open it in your default browser.


Test the form by submitting your information. You should see the following confirmation screen if you did not define a redirect URL when you generated the code. If you did select a redirect, you should be taken to that page.

Now, go back into Listrak to make sure that your information was processed. The subscription may take a few minutes to appear.

With the same marketing list selected in the left-hand corner when the code was generated:

  1. Navigate from the home menu to Contacts.
  2. Then Search for a Contact.
  3. Enter the address you used to test.
  4. Click Search.
  5. Click the email address link to make sure that all of the correct segmentation information was imported.

6. Once you’ve confirmed that everything is passing correctly, you’re done! You can now paste the code onto your site and use the external event in tandem with a Listrak Conductor Conversation to deploy automated messages.
